For all transcript, certificate, diploma, or degree requests:

By mail – Please complete the Transcript Request Form (linked below) and send to the address listed on the form.  The request must include a $20.00 fee for an official transcript or certificate/diploma/degree; there is no charge for an unofficial transcript. Personal checks are not accepted; payment must be in the form of a money order.
